Experienced Leadership

Over the past 15 years, our leadership has helped shape the specialty of hospital medicine. They are widely recognized and respected as industry thought leaders and visionaries. It is their extensive knowledge of the field and leadership roles in its professional societies that provide us with a solid foundation for continuous improvement and innovation.

Integrated Structure

Each Cogent HMG hospital team is vertically integrated with defined roles that support the hospital's operational and clinical functions while remaining focused on achieving the hospital's goals.

Locally, our clinical care teams, consisting of physicians, nurses and administrators, are supported by on site Program Medical Directors. Regional Medical Directors and Operations Vice Presidents provide practice management, planning guidance, leadership development and mentoring. At the national level, our Chief Operating Officer and Chief Medical Officer oversee all clinical and quality initiatives, as well as the corporate infrastructure that supports our local teams.

Coordinated Care Team

Our nurses, physicians and support staff all work together on-site to deliver more efficient and cost-effective care. Our system of care is structured so hospitalists are empowered to focus purely on clinical excellence.

Meanwhile, Clinical Care Coordinators oversee patient safety and care initiatives, documentation and communication both inside the hospital and with referring physicians. Additional support is provided by an on-site Program Manager and Administrative Coordinator.

Ongoing Training

All Cogent HMG employees and physicians periodically attend Cogent HMG Academy™, where they receive further clinical and organizational training. Cogent HMG Academy delivers focused training courses at our dedicated facility in Nashville, as well as a set of online services that give employees access to a consistent training curriculum.

In addition, our physicians have access to a national network of best practices, mentors and information-sharing programs that is unmatched and helps promote consistency in the delivery of care.

Date-Driven Process

At the outset of our relationship with each hospital partner, we carefully align our services with the hospital's clinical, quality, financial and regulatory goals. To measure our success in reaching those goals, as well as to be more consistent in delivery of care, we continuously capture and analyze more than 40 real-time data sources.

Our case-rate compensation model reinforces our commitment to achieving our partner's goals, while our monthly Joint Operations Committee meetings with hospital leadership provide regularly scheduled opportunities to assess performance and fine-tune our program, if needed.
